Psychedelic-assisted Psychotherapy (PAP) is a cutting-edge therapeutic method that  combines traditional psychotherapy with the controlled use of psychedelic substances,  including psilocybin mushrooms, MDMA, LSD, ayahuasca, DMT, and ketamine. This approach follows a structured process that consists of three distinct yet interconnected  phases: preparation, the psychedelic dosing session (often referred to as the “journey”),  and integration. While each phase plays a crucial role in the therapy, the Preparation phase  is especially vital for ensuring a safe and effective treatment.

The Foundation of the Preparation Phase

The Preparation phase is foundational for the success of any psychedelic treatment  protocol. It begins with an in-depth assessment of the client's psychological, medical, and  physical status. This initial evaluation involves gathering a comprehensive survey, both  written and oral, of the client's life experiences and medical history and applying clinical  measurement scales. These scales provide a baseline understanding of symptoms related  to trauma, depression, anxiety, suicidality, substance use and other important  psychological factors. This information is invaluable for identifying potential risk factors  and tracking progress throughout the therapy.

Managing Contraindications and Safety

Certain conditions can contraindicate the use of psychedelic medicines, including  pregnancy, high blood pressure, active substance use, a history of psychosis or bipolar  disorder, and the current use of specific medications such as benzodiazepines. By thoroughly assessing these factors, the PAP team can adjust the client’s treatment plan as  needed to ensure their safety and maximize the therapy's effectiveness.

Crafting the Optimal Set and Setting

Another critical component of the Preparation phase is establishing the “Set and Setting”  for the psychedelic experience. This involves building a supportive, trust-based relationship  between the client and PAP therapist, creating a safe and comfortable environment for the  journey, and helping the client develop a mindset conducive to healing, personal exploration and growth. Setting intentions for the journey is a key part of this process. It  allows the client to articulate a guiding principle or question to focus their awareness  during the journey, enhancing their ability to explore deeper layers of their psyche safely  and purposefully.

Exploring Inner Healing Intelligence

Preparation also provides an opportunity for clients to connect with their Inner Healing Intelligence (IHI), an intrinsic sense that can guide them during their psychedelic  experiences. With the support of their therapist, clients can cultivate a relationship with  their IHI, which helps them build greater self-trust and develop a sense of their own resiliency. This enhanced relationship with Self is crucial for the client to be able to  effectively integrate their experiences throughout the PAP treatment and into the rest of  their life.

Sensory and Experiential Considerations

The Preparation phase may also include discussions about the use of sensory and  experiential elements like music, ritual, and therapeutic touch. Each of these elements can significantly affect the client's sensory experience and sense of safety, playing an essential role in the overall healing process.

Managing Expectations Realistically

Expectation management is a key aspect of Preparation. Clients who have long struggled  with persistent symptoms may be tempted by the notion of a “miracle cure” associated  with psychedelic medicine. It’s important for therapists to provide a realistic perspective on  what the therapy can achieve. This includes discussing the course of treatment, potential  outcomes, and what clients can realistically expect in terms of symptom relief both in the  short and long term.

Practical Logistics and Details

Practical considerations are also addressed during Preparation. This includes advice on  what to wear, dietary recommendations before the journey, and transportation  arrangements, as clients will need to arrange for a ride home post-session. Additionally, clients are informed about the physical, emotional, and psychological effects of the treatment, both during and after their session. These details help clients prepare  eJectively, reducing stress and enhancing their overall experience.

The Necessity of Thorough Preparation

Due to its critical role in ensuring safety, managing expectations, connecting the client with  important aspects of self-support and establishing a healthy and conducive environment for change, the preparation phase takes time and often consists of three or more sessions,  depending upon the individual needs of each client.

Without thorough preparation, the PAP treatment protocol would lack essential nuance,  cohesion and safety considerations, potentially compromising the client’s well-being and  the overall effectiveness of the therapy.

All told, the Preparation phase is intrinsic to the success of any psychedelic-assisted  psychotherapy protocol. It establishes the foundation for a safe, effective, and  transformative therapeutic journey, making it an essential component of the overall  process of lasting change.
